Co-Care Is Not a 911 Emergency Response Dispatcher

Co-Care Health Group Inc. delivers scheduled in-home supportive care and visiting community nursing services. We are NOT an emergency ambulance service, urgent care center, or acute hospital emergency room.

If you, your family member, or our caregiver observes acute, life-threatening symptoms—including chest pain, sudden numbness or paralysis (FAST stroke signs), severe difficulty breathing, unresponsiveness, or catastrophic falls—immediately call 911 or proceed to the nearest emergency department.
Section 01

1. Agreement & Contracting Parties

This Agreement is entered into between Co-Care Health Group Inc. ("Co-Care", "we", "us", or "our") and the individual receiving care, and/or their legally appointed substitute decision-maker, guardian, or power of attorney ("Client", "you", or "Representative").

By signing a Service Agreement, completing an intake assessment, or receiving home care, nursing, or companionship services from Co-Care personnel, you agree to be bound by the terms, clinical standards, and billing policies detailed herein.

Section 02

2. Provision of Services & Nursing Scope of Practice

All clinical and supportive home care services delivered by Co-Care comply with provincial legislation, including The Regulated Health Professions Act (RHPA) of Manitoba:

College of Registered Nurses of Manitoba (CRNM) Standards

Registered Nurses (RNs) perform comprehensive holistic health assessments, advanced wound care, complex nursing procedures, clinical delegation, physician liaison duties, and care plan design in compliance with CRNM Professional Standards for Practice.

College of Licensed Practical Nurses of Manitoba (CLPNM) Standards

Licensed Practical Nurses (LPNs) execute specialized bedside nursing, chronic disease management, specialized diabetic foot care, vital sign monitoring, and medication administration in accordance with CLPNM Competency Standards.

Health Care Aides (HCAs) & Direct Support Workers (DSWs)

HCAs and DSWs operate under direct nursing supervision and clinical care plans to assist with Activities of Daily Living (ADLs)—including bathing, dressing, personal hygiene, meal preparation, mobility assistance, and companionship.

Note: Care staff cannot perform tasks exceeding their legal scope of practice, personal training competencies, or unassigned medical procedures lacking written physician orders.

Section 03

4. Care Assessments & Personalized Service Delivery

Before initiating recurring care, an RN or qualified clinical coordinator will conduct an initial in-person health assessment at the client's residence. This assessment evaluates:

  • Physical, mobility, fall risk, and cognitive support requirements.
  • Environmental and home safety conditions.
  • Medication schedule and prescription administration needs.
  • Client preferences, family caregiver schedules, and goals of care.

From this assessment, an individualized Care Plan is generated. Care Plans are reviewed periodically and adjusted when significant changes in clinical condition occur.

Section 04

5. Client Rights & Client Responsibilities

Client Rights

  • To be treated with dignity, respect, and cultural consideration.
  • To have personal health records guarded under Manitoba PHIA.
  • To participate actively in care planning and schedule selection.
  • To refuse treatment or care within informed legal capacity.

Client Responsibilities

  • Provide accurate medical, medication, and behavioral histories.
  • Maintain a safe, hazard-free, and non-smoking environment for care visits.
  • Secure domestic pets in a separate room during care procedures.
  • Zero-tolerance: No verbal abuse, sexual harassment, or physical threats.

Co-Care reserves the legal right to suspend or withdraw staff immediately if a caregiver's physical safety or human rights are jeopardized in the residence.

Section 05

6. Scheduling, Minimum Hours & Manitoba Winter Weather Protocol

Shift Minimums: To ensure high-quality staff retention and consistent care continuity, community visits require minimum shift allocations (typically 3 to 4 hours per visit, unless otherwise specified for specialized nursing or foot care appointments).

Manitoba Winter Blizzard & Highway Closure Protocol

During extreme Manitoba winter conditions (blizzards, freezing rain, wind chill warnings below -40°C, or RCMP rural highway closures around Steinbach, Winnipeg perimeter, or Eastman region), staff safety is paramount. Co-Care prioritizes critical high-acuity client visits (e.g., life-sustaining medications, palliative care) and coordinates with family contacts or rural emergency responders when travel becomes physically impassable.

Section 06

7. Cancellation, Rescheduling & Shift Protocols

Our care professionals reserve dedicated time specifically for your household. To maintain equitable compensation for our frontline nurses and care aides, the following cancellation rules apply:

24-Hour Advance Notice Required

Clients may cancel or reschedule a scheduled visit without penalty by providing written or verbal notification to Co-Care dispatch at least twenty-four (24) hours prior to the scheduled shift start.

Late Cancellations (< 24 Hours)

Cancellations received with less than 24 hours' notice, or instances where a caregiver arrives and is turned away, may be billed up to the scheduled shift duration (or a minimum 3-hour fee) to cover scheduled staff wages.

Emergency Hospitalization Exemption

Cancellation penalties are waived if a shift cannot proceed due to the client's acute unplanned emergency hospital admission, upon confirmation with the attending health facility.

Section 07

Invoices are due upon receipt or within 15 calendar days as stated on the invoice.

  • Medication Assistance (HCAs):Health Care Aides may assist with pre-packaged blister packs (dosette boxes prepared by a licensed pharmacy), opening bubble packs, handing water, and recording verbal confirmations. HCAs cannot calculate dosages, draw liquids, or administer injectable medications.
